Form A4 should be completed in respect of all operations on blocked accounts, irrespective of whether the operations are made by authorised dealers in terms of Reserve Banks special directions, if any, or require Reserve Banks specific approval. The words BLOCKED ACCOUNT must be prominently written in red ink at the top of every form A4 completed in respect of a debit or credit to a blocked account. Where forms are completed for purpose of obtaining Reserve Banks specific approval, they should be sent to Reserve Bank in duplicate and the debit or credit entry made only after one copy of the form has been returned by Reserve Bank duly approved. Such approved forms A4 as well as forms A4 completed as reports for transactions made in terms of Reserve Banks special directions, if any, should be submitted to Reserve Bank
